Do You Have A Family Plan For Emergencies and Disasters ?
A Family Plan Would Include:
1. Family Information
Information for each family member would include:
Name | Date of Birth | Telephone Number |
Doctor's Name | Social Security Number | Special Medical Information |
Insurance Information | Their Usual Location |

2. Workplace Information/School Information/Day Care Information
Addresses and telephone numbers for each3. Nearest Relative(s) Information
Names, Addresses and telephone numbers4. A plan of how to stay in contact if separated by disaster
Choose two meeting places; primary & secondary5. Choose an out-of-town friend or family member as a contact for everyone to call
Name, address, telephone (home/work/cellular)
6. Designate a room in your home to stay in for several days
7. Designate a place where your family will be able to stay for several days in case of evacuation including the address and telephone number of location or site.
Emergency Kit Checklist:
These items should be part of your emergency kit and kept in a container that can be easily carried.
